Comprehensive program delivers person-centered care
BRI Care Consultation is designed to deliver person-centered care by allowing Care Consultants to help find tailored solutions for virtually any issue experienced by the caregiver or care receiver. Our 35 Assessment Domains, reviewed over the course of the program, evaluate everything from home safety and avoiding falls to concerns about Activities of Daily Living, social isolation, mood swings, transportation, understanding disease progression and much more.
Two delivery models to meet your organization’s needs
BRI Care Consultation can be licensed by your organization and delivered by specially trained Care Consultants on your staff, or Benjamin Rose Institute on Aging can deliver the program to your clients on your behalf through our service delivery arm, WeCare. . . Because You Do.
Licensed Model:

Action Planning helps get other family members involved
One of the unique features of BRI Care Consultation is the Action checklist, which is a comprehensive roadmap of tasks needed to achieve the goals set by the care team. Assigning small and measurable tasks is a great way to get other family members and friends involved in care.
Encourage referrals to your other services
As Care Consultants work with care teams to find solutions to their issues, they frequently make referrals to other programs and services that can provide needed assistance. Many sites find they’re able to at offer referrals to other programs offered by their own organization!
